Telehealth Consult with Dr Ray

As an expert health consultant , Dr Psonak is offering medical Telehealth (phone) consults via appointment.  Schedule a Telehealth consult with Dr. Ray to discuss questions you may have about addressing your health issues. Initial phone consults are scheduled for 30 minutes.  To schedule an appointment please proceed with the following steps:

STEP 1. Complete the following two documents and return them to the office by email, text or fax:

Agreement for Telehealth Appointment

Return above documents by one of the following methods:

NOTE: If you are a previous patient of Dr. Psonak and have not completed the above two forms within the past year please complete them and return them to the office.

STEP 2. Make a payment for the consult.

STEP 3. Schedule a time convenient to you on Dr. Ray’s calendar by clicking here.

STEP 4. Within 24 hours of receiving your agreement, medical history, payment and notice of scheduled Telehealth appointment, we will contact you to confirm your appointment.

STEP 5. At the time scheduled for your confirmed appointment, Dr. Ray will call you (USA Only) at the phone number given on your agreement form.

NOTE: If you have not received an appointment confirmation, your appointment has not been scheduled!

FEES:

Health consults require a minimum 30 minute appointment at a cost of $120.00 or a 60 minute appointment for $240.00  After the initial call, if you wish to continue, we can set up a future medical Telehealth appointment as follows:
30 minutes $120
60 minutes $240

3rd Party release:

If you wish to have Dr. Ray discuss your medical information with a family member or other third party, you will need to fill out and return the “Health Information Release Authorization” form.

Cancellation Policy:

Cancellations are subject to a 5% scheduling if you cancel at least 24 hours prior to your appointment. This 5% fee will be deducted from your refund. There will be NO REFUND on cancellations less than 24 hours prior to your appointment.
